What is Form 5500?
Form 5500 is the annual report that employee benefit plans covered by ERISA file with the U.S. Department of Labor and the IRS. Employer health plans with 100 or more participants must file it each year, reporting how many people the plan covers, how benefits are funded, and, on Schedule A, which insurance carriers are involved, what premiums were paid, and how much brokers earned. The filings are public. This lookup turns them into a readable profile for every self-funded employer health plan with 500 or more participants, and adds StreamRX's estimate of each plan's pharmacy spend.

How this data works
Source
Form 5500 and Schedule A filings for plan years 2024 and 2025, from the U.S. Department of Labor.
Which plans
Single-employer health plans with 500+ participants that report self-funded benefits. Union, trust, and association plans are excluded.
Growth
Year-over-year participant change. Hidden when it more than doubles or halves, which usually means a merger or filing change.
Pharmacy estimates
Participants × the national average commercial drug spend per member, with savings ranges from our savings calculator's default assumptions.
Privacy
No individual names are published. Companies can request corrections at any time.

Every profile is built from the employer's Form 5500 filing and Schedule A insurance reports, which employer health plans with 100 or more participants file each year with the U.S. Department of Labor. The filings are public records.
The filing reports how benefits are paid. We treat a plan as self-funded when benefits are paid from the employer's general assets or a trust and there's no medical insurance contract on Schedule A, or when a stop-loss contract is reported. It's a strong signal, but not perfect.
No. It's an estimate: plan participants times the national average commercial prescription spend per member. Actual spend depends on the plan's claims, and is usually higher once dependents are counted. A StreamRX analysis uses the plan's actual claims.
Most self-funded plans pay claims from company assets, so they aren't required to file the schedule where a PBM would appear. Starting with plan years on or after July 1, 2027, the DOL's new rule requires PBMs to disclose their compensation directly to these plans.
Each profile shows the plan year of the filing it's based on. Filings are due seven months after the plan year ends, so the most recent data typically lags by a year or more. We refresh when the Department of Labor publishes new data.
Yes. Every profile has a 'Request a correction' link. We'll review it against the filing and update or remove information that's wrong.
